Breast mammography and ultrasound lexicon from ACR BI-RADS 5th

1) Tissue composition
  • Homogenous background echotexture – fibroglandular
  • Homogenous background echotexture – fat
  • Heterogenous background echotexture
2) Mass : Shape
  • Oval : mass that is elliptical and egg-shaped (may include 2 or 3 undulations, i.e., gently lobulated or macrolobulated)
  • Round : spherical, ball-shaped, circular or globular (AP diameter equal to its transverse diameter/ must be circular in perpendicular projections)
  • Irregular : neither round nor oval
3) Mass : Margin
  • Circumscribed : well defined with an abrupt transition between the lesion and surrounding tissue
  • Angular : Some or all of the margin has sharp corners, often forming acute angles, but the significant feature is that the margin of the mass is not circumscribed
  • Indistinct : No clear demarcation of the entire margin or any portion of the margin from the surrounding tissue, includes echogenic rim
  • Microlobulated : short cycle undulations
  • Spiculated : sharp lines projecting from the mass
4) Mass : Orientation
  • Parallel : long axis of the mass parallels the skin line
  • Not parallel : the AP dimension is greater than the transverse dimension
5) Mass : Echopattern
  • Hyperechoic
  • Isoechoic
  • Hypoechioc
  • Anechoic
  • Complex solid and cystic (C4) : contains both anechoic (cystic or fluid) and echogenic (solid) components
  • Heterogenous
6) Mass : Posterior features (left – posterior enhancement, right – posterior shadowing)
  • Enhancement : cyst, homogeneous solid lesions (including high-grade carcinomas)
  • Shadowing : Shadowing is associated with fibrosis, with or without an underlying carcinoma.
  • Combined
7) Calcifications (in mass, outside mass, intraductal)
8) Associated features
  • Architectural distortion
  • Duct changes
  • Skin changes : thickening (>2mm), retraction
  • Edema
  • Elasticity
  • Vascularity
Vascularity (Absent, internal, rim)

9) Special cases

  • Simple cyst
  • Clustered microcysts
  • Complicated cyst
  • Mass in or on skin
  • Foreign body (including implants)
  • Intramammary lymph node
  • AVM
  • Mondor disease
  • Postsurgical fluid colleciotn
  • Fat necrosis
