Papillary carcinoma of the breast: Breast cancer which shows complex cystic and solid echogenicity on Ultrasound

  • Intraductal papillary carcinoma
    • intracystic papillary carcinoma (=encapsulated papillary carcinoma)
  • Invasive papillary carcinoma

Older women

Slow growth, good prognosis

Less frequent axillary lymph node metastasis

Imaging finding

  • Mostly circumscribed, complex cystic and solid mass, increased vascularity
  • homogenous echogenicity – uniform internal echo texture
  • intracystic, multiple fronds projecting into the lumen of the cyst (technically this is not cyst, but markedly dilated lumen)
  • intracystic nodule or thickened septations are common

Multiple well defined ovoid to round mass with malignant microcalcifications

Relatively well defined round mass with increased vascularity and surrounded microcalcifications

89 year-old female case) complex solid and cystic mass with prominent vascularity, biopsy confirmed as papillary carcinoma

Contrast-enhanced T1-weighted image of intracystic papillary carcinoma showing marked enhancement of the cystic walls, septations and mural nodules. References: radiology, Korea University Guro Hospital – Seoul/KR
